Community Care Management is only available to DC residents who live in Ward 3 and meet certain financial requirements. There is often a wait list for these services, with priority given to individuals most in need.

Iona Care Managers, who are licensed social workers, are available to:

  • Assess how well an older adult is managing on their own and what care might be needed
  • Arrange and monitor home care and other services
  • Schedule medical appointments and facilitate doctor/patient communication
  • Provide short-term crisis management
  • Apply for government benefits/services
  • Connect individuals to Iona and other community programs and services
  • Provide other supports as needed

Staying at home versus making a move. Coping with physical changes or memory loss. Difficulty driving or managing finances. Paying for care. Transitioning from the hospital or a rehab stay. These challenges can be overwhelming for anyone.

As an independent older adult, you want to age in your home with the comforts and familiarities you know, but that might become increasingly difficult should you ever have an accident or become ill. You might not have anyone to help care for you, or, if you do have family or friends who could help, you don’t want to burden them with your care.

As a caregiver, you may be concerned about helping an older adult in your life and helping them make the best healthcare and lifestyle choices for their situation. You may not be aware of available resources and supports, or you and your family might need guidance in understanding your options and making decisions.

Iona’s licensed social workers and clinical nurse care managers have decades of experience dealing with the challenges of aging — from navigating the complex healthcare system to addressing social isolation and other quality of life issues.

Step 1: Free 15 minute Consultation

Call Iona’s Helpline at (202) 895-9448 and sign up for a free, 15-minute consultation with an Iona care manager via phone or video meeting. Our experts will briefly assess your situation and explain how Iona Care Management can help.

Step 2: Initial Consultation

With your Iona care manager, you will have the opportunity to thoroughly discuss your situation, ask questions, and share concerns or challenges. A consultation typically lasts an hour to an hour and a half, and can take place at Iona, offsite (such as a home or hospital rehab facility), or over the phone. You’ll leave with advice, concrete next steps, and customized referrals that ease your stress and build your confidence.

Step 3: Ongoing Consultation

After your first consultation, you may find that you have additional questions or want to check in with your care manager again. Perhaps your situation has even changed and you need new resources or strategies. Ongoing consultations provide support and reassurance.

Step 4: Care Management

Our expert team has the expertise to put the care plan that you have developed together into action. We research, coordinate, arrange, and oversee services so that you can feel confident and secure that your older adult is receiving the best care possible.

Cost

Iona Care Management is billed privately on a fee-for-service basis at the rate of $195 per hour. Typically, Medicare and other forms of health insurance do not cover care management services, although your Care Manager can refer you to services that Medicare covers.

Have long-term care insurance? Check your policy for benefits which may cover care coordination services provided by our licensed Care Managers.
