As an independent older adult, you want to age in your home with the comforts and familiarities you know, but that might become increasingly difficult should you have an accident or become ill. You might not have anyone to help care for you, or, if you do have family or friends who could help, you don’t want to burden them with your care.

As a caregiver, you may be concerned about helping an older adult in your life and helping them make the best healthcare and lifestyle choices for their situation. You may not be aware of available resources and supports, or you and your family might need guidance in understanding your options and making decisions.

Iona’s Aging and Caregiving Support team has decades of experience dealing with the challenges of aging—from navigating the complex healthcare system to addressing social isolation and other quality of life issues.

Our professional team specializes in working with older adults, family caregivers, and professional associates in navigating and problem-solving the challenges of aging. Our experts know how to resolve the most difficult challenges that come with aging and caregiving.

Step 1: 15-Minute Complimentary Call

Call Iona’s Helpline at (202) 895-9448 and ask to sign up for a 15-minute complimentary phone call with an Aging and Caregiving Support Care Manager. Our team members will briefly assess your situation and explain how Aging and Caregiving Support can help.

Step 2: Initial Consultation

With your Aging and Caregiving Support Care Manager, you'll have the opportunity to thoroughly discuss your situation, ask questions, and share concerns or challenges. A consultation typically lasts 60-90 minutes and can take place at Iona, offsite (such as a home or hospital rehab facility), or over the phone or video. You’ll leave with advice, concrete next steps, and customized referrals that ease your stress and build your confidence.

Step 3: Ongoing Consultations

After your first consultation, you may find that you have additional questions or want to check in with your Aging and Caregiving Support Care Manager again. Perhaps your situation has even changed and you need new resources or strategies. Ongoing consultations provide support and reassurance.

Step 4: Care Management Facilitation

Our team has the expertise to put the care plan that you have developed together into action. We research, coordinate, arrange, and oversee services so that you can feel confident and secure that you or the older adult in your life are receiving the best care possible.

Cost

Aging and Caregiving Support is billed privately. Typically, Medicare and other forms of health insurance do not cover care management services, although your Care Manager can refer you to services that Medicare covers.

Have long-term care insurance? Check your policy for benefits which may cover care coordination services provided by our licensed Care Managers.
