Aging in Place

You want to support an older adult’s wish to continue living in their own home. But how can they do so safely and affordably? Iona's Aging and Caregiving Support team provides solutions such as:

  • Visiting your relative at home to determine if services or home modifications are needed
  • Pointing you to cost-effective home care options
  • Referring you to services on everything from home renovations to social outings and other desired activities

If you do not live nearby or cannot take on the logistics of managing care in the home, an Aging and Caregiving Support Care Manager can also:

  • Arrange and supervise home care services
  • Visit older adults at the schedule of your choosing
  • Troubleshoot problems, including if an older adult is reluctant to accept care
  • Escort older adults to medical appointments
  • Support you in urgent situations or emergencies, including health care crises
  • Communicate with you, family members, service providers, and healthcare providers to ensure an older adult’s needs are met

Hospital/Rehab to Home

When navigating a health crisis, planning next steps can be overwhelming and confusing. Your Aging and Caregiving Support Care Manager works with you, your family, and the hospital’s discharge planner to make an older adult’s transition as smooth as possible. Our safeguarding work ensures that all appropriate services are arranged upon return to home. Our guidance can also reduce the likelihood of re-hospitalization. Aging and Caregiving Support Care Managers can provide assistance with:

  • Consultations on post-discharge logistics, including coordinating with hospital or rehab discharge planners
  • Healthcare advocacy
  • Facilitation and arrangements for whatever an older adult needs at home, including home care services, transportation, meals, equipment, medications, and follow-up medical appointments
  • Evaluation of long-term care needs

Dementia Care
